Practice toolkit
Templates for the business side of an independent, cash-pay practice. They are starting points, not legal or billing advice: check them against current CMS guidance and with your own counsel. Prices are discussed as method, never as agreement.
| Template | What it is for |
|---|---|
| Good faith estimate | What the No Surprises Act requires for uninsured and self-pay patients, and where to get the CMS model form |
| Superbill | An itemized receipt patients can submit to their insurer for out-of-network reimbursement (PDF) |
| Letter of medical necessity | A letter for genetic testing coverage, with places for the indication, the management change and the guideline (PDF) |
| Setting a cash price | A worked method from practice costs to a visit price and a membership fee |
The referral standard is the clinical companion: one question per referral, and a reply within seven days. Suggestions for more templates go to hello@coloradocarenetwork.com or the #practice channel.
