Referral standard
A referral should answer a question. The network asks referring clinicians, and the clinicians on its rosters, to use two one-page forms: a referral that states one question, and a consult reply that answers it first.
The standard
- One question per referral. State it in one sentence and attach the results that bear on it.
- The price is known before the visit. The patient hears the cash price from the roster entry or the clinician's published price.
- The reply comes back within seven days of the visit, answer first. If results are pending, a one-line interim note goes back and the full reply follows.
- The reply says who does what next: the consultant, the referring clinician or the patient.
- Forms travel through the practices' usual secure channel: fax, portal or encrypted email. Never through this website, Slack or ordinary email.
The forms
| Form | What it does | Use it |
|---|---|---|
| Referral | One question, why now, what has been done, what you need back | Web version, PDF |
| Consult reply | The answer first, then recommendations with who and when | Web version, PDF |
Print the PDF or save it and fill it in your own system. Nothing is filled in or sent through this site.
Rostered clinicians
Consultants, specialists, therapists and counselors on a CCN roster agree to reply on the consult form within seven days of the visit. Whether referrals get a reply is one of the measures the network will report on itself, under the quadruple aim. Find a member to send to.
