Specialist
The anchor question: which cash-pay or hybrid specialists will see these patients, work with an anchor physician and write back? The founding journal club's specialist seats come from here too.

Cardiology, neurology and GI are recruited first, because each serves two of the three patient cohorts.
Vetting criteria
- Founded or co-founded the practice, and more than half of its revenue comes from patients rather than payers.
- Publishes consultation prices and will see a patient with an anchor physician's summary in hand.
- Writes back to the anchor physician, not only to the patient.
- Comfortable with a genetic diagnosis in the chart, and willing to attend the cohort journal club.
How the roster is built
- Year one: recruit six to ten specialists, mostly in Colorado, across the first-recruit specialties. Two join the founding journal club.
- Year two: fill the remaining specialties. Open a cross-cohort case conference with the patient-cohort anchors.
- Year three: measure referral turnaround, patient-reported experience, and whether consult notes reach the anchor. Prune and add.
